Licensing Form
Our licensing form is designed so copyright holders can document that they’ve licensed a work under Creative Commons.
Someone who fills out and signs a licensing form represents to the university that they are a copyright holder with the right to license the work specified, and indicates the license they have selected for it.
When to use the form
This form is flexible and appropriate for a wide variety of uses.
For instance, if a university unit plans to host someone's work online, they might ask that person to sign this form first. Or, if an instructor would like the university to be able to use student work produced in a course (such as when students collaborate to prepare an exhibit or an educational resource), they can ask the students to sign this form.
When a licensing form is completed, the party that plans to rely on the license should keep a copy on record.
